        INSTRUCTIONS FOR ADMISSION TO UG COURSE(S) AT M. J. N. MEDICAL COLLEGE & HOSPITAL
Please read the instructions carefully before proceeding for admission –

Physical Documents Verification: Every candidate need to bring the below mentioned documents in Original and One self attested photocopy at the admission front desk.

Online Admission Form Submission: Every candidate must fill up the Student online Admission Form at the admission front desk and collect login credential from the same desk for future reference.

Fees Payment: Total fees to be paid at the time of admission is to be deposited by CASHLESS mode through Debit Card / Credit Card / UPI via POS Machine or in online payment mode through our official website (www.mjnmch.ac.in). Bond: Every candidate must fill up the Bond for resignation and submit the signed copy at the time of admission. Click here to download the Bond for resignation Form.

Medical Certificate: Every candidate MUST bring a Medical Certificate in the given proforma here before reporting for admission. Medical Fitness Certificate is to be issued by Registered Medical Officer. Click here to download the Medical Fitness Form.

Anti-Ragging Undertaking by Student: Every candidate must fill up the Anti Ragging pledge document and submit the signed copy at the time of admission. Click here to download the Anti Ragging Form for Candidate.

(a) SC/ST Certificate issued by the competent authority (in the standard format as specified in the prospectus/information bulletin) and it should be in English or Hindi in language. Sub caste should be clearly mentioned in the certificate. In case the certificate is in regional language the candidate should carry one attested translated copy of the certificate in English/ Hindi.


(b) OBC certificate issued by the competent authority. The sub- caste should tally with the Central List of OBC. The OBC candidates should not belong to Creamy Layer to claim OBC reservation benefit. The OBC certificate must be in the standard format as mentioned in the prospectus/ Information Bulletin. NCL certificate for OBC Candidates need to produce if applicable. (N.B.: Caste Certificate of OBC candidate alloted in UR Quota is not required.)


(c) Disability Certificate issued from a duly constituted and authorized Medical Board for 21 Benchmark Disabilities as per the Rights of Persons with Disability Act 2016 (RPWD Act, 2016). (Please see clause 10.1 of 40 Page Information Bulletin of AIPMT/NEET for admission to MBBS/BDS Courses, published by Central Board of Secondary Education (NTA) for details). No other PWD certificate, issued by any other Authority/Hospital will be entertained.



N.B.: Original 10+2 Marksheet, 10+2 Pass Certificates, Migration/School Leaving Certificate (for University Registration), Medical Certificate, Domicile Certificate (for State quota) will be retained by the college authority against a receipt which has to be retained carefully for withdrawal of original documents at completion of course. For submission of these documents please bring a transparent plastic folder with name of the candidate and NEET Roll Number labeled.
